Waiver and release of  liability

 

As the parent or legal protector of the named player, I recognize that I am wholly aware of the intrinsic risks that might be related to soccer. Therefore, as a result of this, we waive the release and emancipation of Ole Soccer Academy and as well as their officers, agent, directors, and collaborators from any accountability and responsibility if the player or I become injured in any way during our contribution to soccer practices, events or activities coupled with the released parties.

I knowledge, appreciate, and assume the risk of participating in sports programs and of physical engagement generally, including but not limited to falls, contact with or effects of meeting with other participants, contact with or results of the use of sporting equipment, effects of weather, including heat, humidity, wind, cold and precipitation, defective equipment, and any hazard that spectators or volunteers may pose.

I allow Ole Soccer Academy to take harmless pictures without identifying my child during the practices and post them on the website/social media only for illustration purposes. These activities must comply with the California Department of Public Health (CDPH) and CDC guidelines.

- Face coverings that cover the nose and mouth are optional for coaches/instructors/players.

- The parents should stay near the location to assist their child if needed. 

- It is recommended that all participants bring their equipment (soccer ball).

- Anyone currently experiencing fever, cough, shortness of breath, chills, muscle aches, sore throat, runny nose or congestion, loss of taste or smell, nausea, vomiting, or diarrhea is not permitted to participate and should be sent home immediately.

- Hand sanitizer should be made readily available at the training site.

- Participants should bring their water bottles and not share them with others.

We can only provide one makeup class per month per batch if your child misses one or more practices within the month. This class should be scheduled through the link on the website. You have 30 (thirty) days to request your makeup class related to the ongoing month or the month before. * It is not applied to futsal groups.
We will not provide a makeup class for weather circumstances requiring the class to be canceled for safety.

I have read the above waiver and release, understand, and signed it voluntarily.
